YOUR FIRST DENTAL VISIT

During your initial dental appointment, we will strive to get to know you as a patient, perform diagnostic x-rays, comprehensive photos, a hygiene exam, and one of our doctors will do their through exam. In some cases, treatment can be diagnosed that same day, though a complex medical history or treatment plan may require an evaluation and a second appointment to provide the best care. For the best experience, please allow for one to two hours of time for your first visit with us.

To help us serve you, please provide the following information prior to your visit:

• An updated medication list

• Your Dental Insurance and photo ID

• Your referral slip and any prior X-rays (if applicable)

IMPORTANT: A parent or guardian must accompany all patients under 18 at the consultation visit.

Please alert the team at Maxwell Creek Family Dentistry if you have a medical condition that may be of concern prior to surgery (i.e. artificial heart valves and joints, diabetes, high blood pressure, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, blood thinners, aspirin, etc.).
